This is a horror novella about the dangers of doing anything and everything to try to maintain one's appearance - the equivalent of facelifts, tummy tucks, liposuction, dental veneers, etc. - but accomplished by expensive, painless nanomachines which alter the body's attributes for maximum health, fitness and beauty.
As long as you can pay. And pay. And pay.
And if you can't pay - well, then it's back to the trash heap for you.
This is a pointed commentary on our current appearance-focused society, but it's an extremely grim one, and I don't recommend it to anyone but the most mentally-healthy readers, because it is brutal. Readers who have ever had to deal with crushing debt may find this one too hard to bear.
